Community Needs Assessment to Identify Regional Healthcare Gaps

ELKINS, WV – Davis Medical Center and Broaddus Hospital are currently conducting Community Health Needs Assessments. The assessments are a requirement of the Patient Protection and Affordable Care Act, and will provide information to be used to assess and improve local access and delivery of health-related services.
The anonymous survey asks participants to answer questions about access to and use of health care, personal behavior and risk factors, and social determinants of health. The results will guide Davis Health System by identifying key health problems and strategies to address these challenges and deficiencies.
The surveys are available electronically on the Davis Health System Face book page at www.facebook.com/dhs.wv. Print copies are also available in the Administration offices of Broaddus and Davis Medical Center.
The survey takes less than 15 minutes to complete. The deadline to complete is Wednesday, October 26. The final community health needs assessment report will be published in 2017 and will be used to prioritize needs and create a community health improvement plan.
Completed paper surveys can be mailed back to Davis Medical Center Marketing Department, 812 Gorman Avenue, Elkins, WV 26241, or returned to the location where they were picked up.
